This website was designed to assist your progress through becoming a qualified Non-Teaching Substitute in our district.  It provides standard documents that must be completed before your applicant and employment information can be processed and entered into our systems.

Once you have completed each of the documents indicated on the following pages (most forms can be typed electronically and then printed), please return your documents to:

General Materials		Tax Forms
	Application Completion Checklist Are your application and employment materials complete?  Please print this checklist to verify that you have completed the necessary information.		Federal W-4 Tax Withholding Allowance Form Federal tax form used in allocating withholding allowances.  Failure to provide a properly completed form will result in your being treated as a single person who claims no withholding allowances; providing fraudulent information may also subject you to penalties.
	Federal I-9 Employment Eligibility Verification Form Federally required documentation of employment eligibility.  NOTE:  Present this form, along with original forms of acceptable documentation, to an authorized GCPS employee at the GCPS Division of Human Resources located at: 437 Old Peachtree Rd, NW Suwanee, GA 30024 You must also bring a copy of your Social Security card and Drivers License with you.  The copies will be retained by GCPS.  Copies can be made on-site for a fee.		Georgia G-4 Tax Withholding Allowance Form Georgia tax form used in allocating State income withholding tax allowances.  Failure to submit a properly completed Form G-4 will result in your employer withholding tax as though you are single with zero allowances.

	Direct Deposit Authorization Form Use this form to authorize Payroll to directly deposit your pay into a designated account.		Substitute Compensation Data Form This document provides GCPS information required to prepare your employment status in our payroll and employment system.  Please return your Data Form to Human Resources along with all the other required materials listed on this site.
	Gwinnett County Public Schools Policies All employees of Gwinnett County Public Schools are required to be familiar with and abide by the policies and procedures established by the Board of Education.  Please read those contained behind this link and complete the required acknowledgement form.		Part-Time Seasonal and Temporary Employees Alternative to Social Security Part-Time, Seasonal and Temporary (PST) employees are covered by a private pension plan established with the provision of the Omnibus Budget Reconciliation Act of 1990.
